Child
- Full name and the name they go by
- Date of birth and grade
- Program, session or days they're signing up for
- Language spoken at home, if it helps staff
Parents or guardians
- Names, phone numbers and email addresses
- Who receives messages and who pays
- Custody arrangements or court orders staff need to know about
Emergency contacts
- At least two adults besides the parents, with phone numbers and their relationship to the child
Authorized pickup
- Every adult allowed to pick the child up, with phone numbers
- Anyone who is not allowed to pick the child up
- How staff will confirm who they are: photo ID the first time, or a pickup code
Health information
Limit who can see health information to the staff who need it.
- Allergies and how serious they are
- Medications, and whether staff need to give any during the program
- Medical conditions staff should know about, and what to do in an emergency
- Doctor's name and phone number, and insurance details if your program needs them
Permissions and agreements
- Photo and video release
- Field trips, swimming and transportation
- Sunscreen and bug spray
- Your participation agreement or waiver, signed by a parent
- Your payment and refund policy
Keep it short
- Ask only what you'll use. Every extra question makes the form harder to finish on a phone.
- Let returning families reuse last season's answers and just confirm them.
- Collect everything before the first day, not at drop-off.
Check your own rules
This checklist isn't legal advice. Check your state's licensing rules and your insurer's requirements, and have a lawyer review your waiver.
How BackPR handles it
In BackPR, families fill out forms and sign waivers online as part of registration, in English or Spanish, and returning families don't have to type everything again. Staff check children in and out against each family's authorized pickup list.
