Tips

How to use it well

  • Send it with registration or the welcome letter, and ask for it back a week before camp.
  • Give counselors a short summary, not the full forms, and keep sensitive details with the director.
  • Follow up with a call when a family mentions a need you'll have to plan for.

Rules on ratios, medication and reporting vary by state. See your state in our state-by-state rules for youth programs.

FAQ

Questions, answered

Why ask families to fill out a camper questionnaire?

It helps counselors connect with each child from the first day, spot worries early and plan support, especially for first-time or younger campers.

Who sees the answers?

Only the staff who need them, such as the child's counselors, directors, and health or inclusion staff. Say so on the form.
