Camp bus and transportation permission form

Families say how their child gets to and from camp: bus stop, carpool, walking or biking home alone, and who may meet the bus.

What's in it

What the transportation permission form covers

At the top: Please tell us how your child will arrive and leave each day. If plans change on any day, tell us in writing by [time]. We never accept a change from a child alone; we confirm it with you.

Camper

  • Full name
  • Age
  • Session and weeks

Morning

  • Dropped off by a parent or authorized adult
  • Camp bus
  • Carpool
  • Walks or bikes to camp
  • Bus stop (if using the camp bus)

Afternoon

  • Picked up by a parent or authorized adult
  • Camp bus
  • Carpool
  • Walks or bikes home alone (see below)
  • Bus stop (if using the camp bus)
  • Adults who may meet the bus (must also be on the authorized pickup list)

Carpool

Carpools are arranged by families, not by the camp. The camp releases your child only to drivers you list here, and they must also be on your authorized pickup list. Drivers are responsible for car seats and booster seats your child needs.

  • Drivers in the carpool (names and phones)

Leaving alone

Only for campers age [age] and older, and only if your state's rules allow it.

  • My child may walk or bike home alone, and I accept responsibility for them once they leave camp

If no one meets the bus

Your child stays on the bus and returns to camp, and we call you. [Late pickup fee, if any.]

Signature

  • Parent or guardian printed name
  • Parent or guardian signature
  • Date

Free to use and change, but not legal or medical advice. Check your state's licensing rules and your insurer's requirements, and have a lawyer review any consent or waiver language.

Tips

How to use it well

  • Give bus counselors a printed list of who may meet each child at each stop.
  • Take attendance when campers get on and off the bus, and check that the bus is empty at the end of every route.
  • Ask your insurer and check state rules before you let campers leave alone or ride with other families.
  • Keep this form with the authorized pickup form so staff check one list.

Rules on ratios, medication and reporting vary by state. See your state in our state-by-state rules for youth programs.

FAQ

Questions, answered

Do camps need permission for carpools?

It's good practice to get a parent's written permission naming the drivers, so staff know which adults may take a child. Check your state rules and insurance before you arrange carpools yourself.

What happens if no one is at the bus stop?

Most camps keep the child on the bus, bring them back to camp and call the parent. Put your rule on the form so families know ahead of time.
