Emergency contact form
A one-page form for each child: parents, up to three emergency contacts, doctor, insurance and the health details staff need when something goes wrong.

Free to use and change, but not legal or medical advice. Check your state's licensing rules and your insurer's requirements, and have a lawyer review any consent or waiver language.
Tips
How to use it well
- Keep a copy with each group's roster and in every field trip bag, not only in the office.
- Ask families to check and update the form at the start of each session or season.
- Call the contacts on the first day for any child whose numbers look wrong or incomplete.
- Lock up the copies you aren't using. The form holds health and insurance details.
Rules on ratios, medication and reporting vary by state. See your state in our state-by-state rules for youth programs.
FAQ
Questions, answered
How many emergency contacts should a camp or childcare program ask for?
Most programs ask for at least two adults besides the parents, and some states set a minimum. Ask for people who live or work nearby and can answer during program hours.
Can an emergency contact pick up a child?
Only if the parent also lists them as an authorized pickup. Keep the two lists separate so staff know who to call and who may take a child home.
