Every camp, team and after-school program asks for the same details. Fill them in once here: allergies, medications, emergency contacts and who can pick up. Print a summary to hand in or copy from when you fill in a program's forms, or save a file you can update next season.

### How it works

**Save a file for next time** downloads your answers in the [Open Youth Registration format](/open-youth-registration), a free open standard. Open it here next season to update it. To hand in your details, print or save as PDF.

**Remember my answers** keeps a copy in this browser and updates it as you type. Turn it off or select Start over to delete it. Leave it off on shared computers.

**Will programs accept this instead of their own form?**
Usually not. Most programs need their own signed forms. Use your summary to fill them in faster, and hand it in as an extra page if the program allows it.

**Is my child's information safe?**
It stays on your device. It only goes elsewhere if you hand in a printout or send someone the PDF or file. Keep your saved file somewhere private, and leave Remember my answers off on shared computers.
