> We take photos and video of program activities to share with families and to tell people about our work. You choose where your child may appear. Your answer won't affect your child's place in the program.

**Child**
- Full name: ______
- Program and session: ______

**Where we may use photos and video of your child**
Check each one you agree to. Leave a box blank to say no.
- [ ] Private updates to families in my child's program (such as emails and newsletters)
- [ ] Printed materials, such as flyers, brochures and reports to funders
- [ ] Our website
- [ ] Our social media accounts
- [ ] Local news stories about the program
- [ ] Paid ads for the program

**Your child's name and work**
We never post a child's full name, school or home address with a photo. News outlets make their own choices, so we'll ask you before a reporter interviews or names your child.
- [ ] You may use my child's first name with photos
- [ ] You may display and share my child's artwork, writing and projects

**Changing your mind**
This permission lasts until [the end of this program year] unless you change it. To change it, tell us in writing at [email]. When you ask, we'll stop using photos of your child and remove them from our website and social media accounts within [number] days. We can't take back items that are already printed or that others have shared.

**Signatures**
- Parent or guardian's printed name: ______
- Parent or guardian signature: ______
- Date: ______
- Participant signature (for teens, optional): ______

### How to use it
- Keep a 'no photos' list with each group's roster so staff know which children to leave out of shots. Put any child whose form isn't back on the list.
- Check the list before you post anything, including group photos where a child is in the background.
- Post after an event, not during it, and leave out details that show where children are right now.
- Ask again each program year, and update the list as soon as a family changes their answer.

**Do we need a photo release to post pictures of kids in our program?**
Most youth programs get written permission from a parent or guardian before using a child's photo, especially online and in promotional materials. Families expect it, and rules vary by state. Have a lawyer or your insurer review your wording if you're unsure.

**What if a parent says no to photos?**
Respect it fully. Add the child to a 'no photos' list kept with the roster, keep them out of group shots you plan to share and check before posting. A family can say no without it affecting their child's place in the program.

Free to use and change, but not legal or medical advice. Check your state's licensing rules and your insurer's requirements, and have a lawyer review any consent or waiver language.
