### How to use it
- Send it with registration or the welcome letter, and ask for it back a week before camp.
- Give counselors a short summary, not the full forms, and keep sensitive details with the director.
- Follow up with a call when a family mentions a need you'll have to plan for.

**Why ask families to fill out a camper questionnaire?**
It helps counselors connect with each child from the first day, spot worries early and plan support, especially for first-time or younger campers.

**Who sees the answers?**
Only the staff who need them, such as the child's counselors, directors, and health or inclusion staff. Say so on the form.

Free to use and change, but not legal or medical advice. Check your state's licensing rules and your insurer's requirements, and have a lawyer review any consent or waiver language.
