> Write every health visit here, even small ones. If you give medicine under a parent's medication authorization, also record it on the medication administration log. Use the incident report for injuries that need follow-up.

**Day**
- Date: ______
- Health staff on duty: ______

**Visits**
| Time | Camper | Group | Complaint or injury | What you saw | Care given | Medicine (name, dose) | Family called (time) | Outcome | Initials |
| --- | --- | --- | --- | --- | --- | --- | --- | --- | --- |

### How to use it
- Keep the log in the health center and take a copy on trips away from camp.
- Look for patterns: several stomachaches in one cabin, or many bee stings near one trail.
- Store completed logs as your state and insurer require. Health records are private.
- Check your state's camp rules for what the health log must include.

**What is a camp health log?**
A dated record of every health visit at camp: who came in, why, what care or medicine was given and whether the family was called.

**How long should camps keep health logs?**
It depends on your state's rules and your insurer. Many keep them for years, because injury claims for minors can be filed long after camp. Ask your insurer.

Free to use and change, but not legal or medical advice. Check your state's licensing rules and your insurer's requirements, and have a lawyer review any consent or waiver language.
